In compliance with USA Hockey, will post on this website volunteers who have successfully passed screening and background check. The screening program includes all volunteers who have routine access to children (this includes but is not limited to Coaches, Asst. Coaches, Team Managers, and Trainers). Many screening forms were returned unprocessed because of errors. Before you send your screening forms to Missouri Hockey please be sure to:

Complete all forms.
Have the form notarized.
Include a $20.00 check or money order made payable to Missouri Hockey.
Place the forms in an envelope and seal it.
State on the outside of the envelope:
